Gastrointestinal (GI) and hepatobiliary disorders affect the structure and function of the GI tract. Many of these disorders often have similar symptoms, such as abdominal pain, cramping, constipation, nausea, bloating, and fatigue. Since multiple disorders can be tied to the same symptoms, it is important for advanced practice nurses to carefully evaluate patients and prescribe a treatment that targets the cause rather than the symptom.
Once the underlying cause is identified, an appropriate drug therapy plan can be recommended based on medical history and individual patient factors. In this Assignment, you examine a case study of a patient who presents with symptoms of a possible GI/hepatobiliary disorder, and you design an appropriate drug therapy plan.

Welcome to Week 4! Hope you had a great weekend.

As an APRN, you will likely encounter patients presenting with symptoms affecting the GI tract. However, most symptoms concerning the GI tract are non-specific and therefore, diagnosis requires thoughtful and careful investigation. Similarly, hepatobiliary disorders may also mirror many of the signs and symptoms that patients present when suffering from GI disorders.

How might you tease out specific signs and symptoms between these potential disorders and body systems? What drug therapy plans will best address these disorders for your patients?

Patient HL comes into the clinic with the following symptoms: nausea, vomiting, and diarrhea. The patient has a history of drug abuse and possible Hepatitis C. HL is currently taking the following prescription drugs:

· Nifedipine 30 mg daily

· Prednisone 10 mg daily

Hepatitis C is among the diseases that are associated with liver complications such as the cases of cirrhosis or inflammation. Before intervention, it is imperative that the history of medication for patient HL be determined considering that it might relatively affect the measures necessary for treatment. For example, the use of Prednisone implies that the patient’s immune system is being suppressed which leads to the determination of a causative factor for such a prescription. As such, the possible diagnosis that would be relevant for patient HL is intestinal obstruction, Crohn disease, or the irritable bowel disease. This is because such conditions are associated with the symptoms of fatigue, nausea, and anorexia. Based on the patient’s historical information, various tests have to be conducted including; the stool test, abdominal X-ray, h.pylori test, and the baseline one (Hirschfield et al., 2015 – Research Paper Writing Help Service). The most likely diagnosis for patient HL is acute gastroenteritis whose symptoms include nausea, fever, abdominal cramps, and vomiting.
The first intervention measure is the maintenance of good hygiene since the disease is transmitted through the ingestion of contaminated fluids or food stuffs. Some medication could be prescribed such as 8mg ondansetron that should be consumed thrice in a day as it is metabolized by the liver (Ibrahim et al., 2016: 2024 – Do my homework – Help write my assignment online). Furthermore, patient HL will be obliged to consume significant amounts of treated water and fluids to replace the lost amounts. In the event that the diarrhea persists, the patient can be given loperamide which also means that they would have to stop taking prednisone.
Due to the withdrawal of prednisone, there is a high chance of upsetting the GI in which the adrenal gland that is produced by the cortisol hormone could embark on its normal functioning (Syzmanski & Szajewska, 2017). To avoid further damage to the liver based on the possibility of patient HL having hepatitis C, the dosage should be allocated in relevant amounts by appropriately adjusting them.
