a. The component of the nurse-patient relationship that the nurse violated was trust. The patient had a trust with the nurse, believing that the nurse will keep her world and she is reliable on not informing the mother on the reason why the patient was admitted in the hospital. However, the nurse informed the mother on what happens for John to be admitted. As an adult patient, John has the right to decide whom to disclose his health information to and the nurse should respect the patient’s decision (Miller & Videbeck, 2017). Disclosing John’s information jeopardizes his safety and trust toward the healthcare system.
b. The behavior of taking personal calls while attending the patient violates the therapeutic relationship between the nurse and the patient. The nurse depicted her unprofessionalism which violates the contract between her and the patient. Having a personal phone call while attending patient contradicts the nurse’s decision of application of treatment plan. Sharing of John’s information with the mother indicates a violation of a patient’s confidentiality.

a. “What should we talk about today” is an appropriate opening statement as it provides a sense of inclusion to the patient on the diagnosis process. The patient can feel like part of the diagnosis process and therefore feel free to interact with the psychiatric nurse throughout the process. The statement creates a friendly atmosphere thus enhancing the relationship between the nurse and the patient which basically makes psychological therapy easy.
b. The nurse is required to use simple, concrete, and direct message while communicating with the patient to enable a clear understanding of the patient. Using difficult and complicated terms and language may confuse the patient and end up not helping him. A good communication process is whereby there is a clear understanding among the participants (Miller & Videbeck, 2017). The nurse, therefore, should ensure the patient understands the therapeutic language to encourage him to participate in the provided psychological therapy process.
